Hemodynamic Response: Decrease in Cardiac Output vs Reduction in Vascular Resistance

چکیده

From a hemodynamic point of view, an adequate response to antihypertensive therapy would be restoration of a normal circulatory system. In most patients with mild to moderate essential hypertension considered to need drug therapy, the cardinal hemodynamic disturbance is an increased total peripheral resistance (TPR) and a normal or reduced cardiac output (CO). During a 10to 17year follow-up of untreated hypertensives, a gradual increase in TPR, increase in MAP, and a decrease in CO and stroke volume (SV) were seen. Hemodynamic responses to chronic drug therapy were studied at rest and during exercise in 250 men with mild to moderate essential hypertension in WHO Stage I. A significant reduction in TPR was seen on thiazide diuretics, nifedipine and verapamil, but there was no increase of subnormal CO or SV. A greater normalization of central hemodynamics was achieved by prazosin, which induced a reduction in TPR and an increase in CO and SV, particularly during exercise. In contrast, beta-blocker therapy was associated with a chronic reduction in CO and heart rate (HR) and usually no reduction in TPRI below pretreatment values. The chronic CO reduction was associated with an increase in arteriovenous oxygen difference. In 14 patients with therapy-resistant hypertension, a marked increase in TPR was found. Captopril induced a reduction in TPR with rest and exercise, and also a reduction in cardiac output. Prolonged therapy for 5 years with beta-blockers did maintain blood pressure control, but with no further decrease in TPR. In contrast, 5 years of therapy with labetalol induced a gradualfall in TPR below pretreatment levels, and a gradual increase in CO. (Hypertension 5 (supp til): 111-49— Hf-57, 1983)

Adrenergic vasoconstriction as a cause of inadequate hypotensive response to beta-adrenergic blockade.

This overview is concerned with the causes of nonresponsiveness to the hypotensive action of beta-adrenergic blocking drugs. The overall hemodynamic response, i.e., a secondary decrease in peripheral vascular resistance, is unrelated to the primary decrease in cardiac output. The reduction in vascular resistance may be triggered by mechanisms residing in the central nervous system, the arterial...

328 SLEEP IS A COMPLEX AND HETEROGENEOUS STATE THAT ACCOUNTS FOR A SIGNIFICANT FRACTION OF THE BEHAVIORAL PATTERN EXHIBITED DURING A 24-hour period.1 The 2 states of natural sleep, namely slowwave or synchronized sleep and rapid eye movement (REM) or desynchronized sleep depend on distinct neural mechanisms and affect the regulation of the cardiovascular system differently.2 Hemodynamic control...

Evaluation of Cardiac Function in Patients with Brain Death using Advanced Hemodynamic Monitoring

Background: Studies on hemodynamic changes in brain death are in vitro and in animal studies. And very few studies have been done on the hemodynamic changes of brain death. The aim of this study was to use advanced hemodynamic monitoring with echocardiography for evaluation of donated heart and to evaluate the moment by moment brain death patients with advanced hemodynamic monitoring tools and ...

The Hemodynamic Response

To determine whether the hyperkinetic circulatory response to chronic anemia is obligatory and to assess its strength and potential reversibility, the hemodynamic state at rest was assessed (indicator-dilution method) in 24 patients with chronic anemia before and after certain interventions. A comparison of cardiac output measured before and after treatment established the consistency of the hy...
